The situation

Keep the doctor's name in the Latin spelling patients already know, or in Devanagari if that is how the family reads it. Mixing both in one line is a common place the voice hesitates. Paste Unicode Hindi directly. A photo of a paper note is not readable here.

The reading

Written language English, spoken language Hindi, unless the reminder is already in Hindi and should only be spoken. Aap is safer than tu for a patient. Put that in the sentence. Studio and Mood voices do not speak Hindi, so clone only with a quiet sample and permission.

What to correct

Times and names fail first. If 4:30 becomes a different hour, rewrite the time line and speak the script again. The polite opening can remain in the script. Download the audio for the handset.
